Can You Do IVM if You Have PCOS? What the Research Shows
Getting a diagnosis of polycystic ovary syndrome alongside a fertility diagnosis can feel like being handed two problems instead of just one. Many women in this position ask the same question early on: is in vitro maturation even an option, or does PCOS rule it out before treatment can start?
The short answer from in vitro maturation research is that PCOS does not close the door on IVM. In fact, PCOS is one of the conditions this technique was designed to help. At Chedid Grieco, we work with women who have PCOS regularly, and IVM was created with exactly this kind of patient in mind.
Why PCOS Makes Standard IVF More Complicated
PCOS causes the ovaries to carry a high number of follicles, which means they tend to respond strongly, sometimes too strongly, to the hormone injections used in conventional IVF. That heightened sensitivity raises the risk of ovarian hyperstimulation syndrome, a painful and occasionally serious complication where the ovaries swell and fluid shifts into the abdomen.
This is not a minor side effect to plan around. For many women with PCOS, it is the single biggest reason a fertility team hesitates before recommending standard stimulation protocols. We take this risk seriously, which is part of why we started offering IVM as an alternative path for candidates who qualify.
How IVM Changes the Equation for PCOS Patients
IVM retrieves eggs while they are still immature, then matures them in a lab setting instead of inside the body. Since the ovaries are not pushed to produce a large batch of mature eggs through weeks of injections, the ovarian hyperstimulation syndrome (OHSS) risk that worries so many PCOS patients drops substantially.
A 2025 narrative review published in PMC described IVM as particularly beneficial for patients at risk of OHSS and for those with PCOS specifically, noting that it offers a less invasive alternative to conventional ovarian stimulation. That is not a small claim, and it means the technique was built around cases like these rather than being adapted after the fact.
What This Looks Like Day to Day
In practice, you’ll have far fewer injections, a shorter runway before retrieval, and less physical strain during the process. We built our program precisely around this tradeoff so PCOS patients are not stuck choosing between avoiding OHSS and pursuing a pregnancy at all.
What Makes Someone a Good Candidate
Not every PCOS diagnosis looks the same, and candidacy depends on a few specific factors we review during a consultation rather than a blanket rule. Several signs tend to point toward IVM being a strong fit for a particular patient, including:
- High antral follicle count: A large number of small follicles visible on ultrasound is common in PCOS and is one of the clearest indicators IVM may suit a patient well.
- Prior OHSS history: Anyone who has experienced hyperstimulation before is often steered toward IVM to avoid a repeat episode.
- Injection sensitivity: Patients who struggle physically or emotionally with a 10- to 12-day injection schedule may prefer the shorter IVM timeline.
We talk about these factors individually with each patient rather than applying a generic standard to all, since PCOS presents differently from person to person.
How Chedid Grieco Approaches IVM for PCOS Patients
Dr. Chedid brought this technique back after training in Vietnam, where IVM protocols have been refined specifically to serve patients who can’t safely undergo heavy ovarian stimulation. That background shapes how we screen and prepare every PCOS candidate who comes through our doors.
Consultations happen in Miami, while the treatment itself takes place in our São Paulo, Brazil facility, giving patients access to a dedicated human reproduction laboratory built around advanced retrieval and maturation techniques. We also guide PCOS candidates through what to expect from the recovery period after an IVM retrieval compared to traditional IVF, since the shorter injection window often means a noticeably easier physical recovery too.

PCOS does not have to be the reason a fertility plan stalls before it starts. With nearly 8,780 babies born through our program, 1,200 cycles performed each year, and 30 years of combined experience, our team has spent decades refining approaches like IVM for patients who need an alternative to standard stimulation.
We hold consultations in Miami and carry out treatment in Brazil through fertility tourism, and our clinic is licensed by both the FDA and the New York Department of Health, giving U.S. patients an added layer of confidence when treatment happens abroad.
