Hysteroscopy Before IVF: When It’s Recommended and Why

Doctor or psychologist with patient couple consulting on marriage counseling, family medical healthcare therapy, fertility treatment for infertility, or psychotherapy session concept

Uterine cavity issues can quietly interfere with embryo implantation, even when every other part of a fertility workup looks normal. If you have gone through one or more failed IVF cycles without a clear explanation, a closer look inside the uterus may be the missing piece.

At Chedid Grieco, we use hysteroscopy to examine the uterine cavity directly, giving us a clearer picture of your reproductive health before you begin or repeat an IVF cycle. Our team combines advanced diagnostic tools with individualized treatment plans so every recommendation is based on what your body actually needs.

What Hysteroscopy Involves

Hysteroscopy uses a thin, lighted scope inserted through the cervix to let a doctor view the inside of the uterus in real time. The procedure can identify polyps, fibroids, scar tissue, or structural irregularities that an ultrasound alone may not catch.

Many patients complete the procedure in a single outpatient visit with minimal downtime. If an abnormality is found, we can often address it during the same session, which may reduce the number of appointments needed before moving forward with treatment.

When We Recommend It Before IVF

Hysteroscopy is not automatically required for every IVF patient, and current research remains mixed on its value for women with a normal ultrasound and no prior failed cycles. A large randomized trial found that routine hysteroscopy did not improve live birth rates in that specific group. That said, several other studies point to real benefits in certain situations, and our recommendations are based on your individual history rather than a blanket policy.

We are more likely to recommend the procedure when you have experienced any of the following:

  • Recurrent implantation failure: Two or more unsuccessful embryo transfers with no clear cause identified.
  • Abnormal imaging results: An ultrasound or other scan that suggests a possible polyp, fibroid, or irregular cavity shape.
  • Unexplained infertility: Difficulty conceiving for an extended period with no diagnosed reason.
  • A history of uterine surgery: Prior procedures that could have left scar tissue behind.

If any of these apply to your situation, addressing a uterine issue before your next transfer may improve your chances of a successful outcome.

How This Fits Into Your IVF Plan

We view hysteroscopy as one piece of a broader, personalized approach to your treatment plan. Rather than applying the same protocol to every patient, we review your history, prior cycles, and imaging results together to decide whether the procedure makes sense for you.

For some patients, we also introduce in vitro maturation as an alternative or complementary option within their treatment plan. This technique retrieves eggs without ovarian stimulation, which can reduce the injection burden many women experience during IVF or egg freezing. Every recommendation we make, including whether to add a hysteroscopy, comes from the same commitment to individualized, boutique care.

Why Timing Matters

When hysteroscopy is recommended, we typically perform it in the cycle just before your IVF treatment begins, rather than months in advance. This timing keeps the uterine environment as close as possible to how it will appear during your embryo transfer, and it means the procedure will not add significant delay to your overall plan.

Coordinating this timing closely with your IVF cycle also helps us catch and treat findings before they can affect embryo implantation. This dual step of evaluating and correcting in one process is part of why several studies have linked hysteroscopy to improved outcomes in patients with a history of failed cycles.

Building Your Family With Chedid Grieco
